The following forms for Minor Modifications are available. Each form includes a summary of relevant procedures and guidelines.
Governance Form A: Minor Modifications to Graduate Curriculum [Word]
- In addition, a revised Calendar entry based on the currently-approved version in the Calendar (available from the SGS website) is needed for most proposals.
Governance Form B: Minor Modifications to Courses [Word]
- In addition, proposals involving courses that are required to complete a graduate program require a revised Calendar entry based on the currently-approved version in the Calendar (available from the SGS website).

Governance Form D: Minor Modifications to Participation of Programs in Collaborative Program [Word]
SGS Academic Activity (ROSI) form [Word]
- Required when submitting some proposals (it is included with forms B and C above, but also available separately for convenience).
Addendum to Memorandum of Agreement (MoA) for collaborative programs [Word]
- Required when adding participation of a degree program to a collaborative program (it is included with form D above but also available separately for convenience).
